Your first visit to Ponte Vedra Premier Dental is designed to get you better acquainted with all we offer as well as introduce you to Dr. Winter or Dr. Neal and our attentive staff. We encourage questions and do our best to always deliver quality care. Approximately three days before your first appointment, we will send you an email request to fill out e-forms online at a secure website. Please take a moment prior to your scheduled appointment to fill out your patient forms. We ask that you come to your appointment 10 minutes early to complete signing off on the forms.
